If you wish to file a claim against the City of Oakdale, please complete a Claim Form provided below, attach any receipts or estimates, and submit to City Hall, (280 N. Third Avenue, Oakdale, CA 95361) within six months of date of incident.  Your claim will be reviewed and forwarded to the City Administrator and/or insurance adjuster for processing.  You may also receive a claim form by requesting one from the City Clerk at 845-3571 or address your request to: City Clerk, 280 North Third Ave., Oakdale, CA 95361.
